Client Assessment and Care Planning

IMT Care conducts initial assessments of clients needs and their preferences to develop personalized care plans. This process would involve understanding clients’ health conditions, lifestyle requirements, family involvement, and any specific preference that may be requested for each clients personalized plan. We also offer Care Coordination by a regulated professional.

Support Services

Our certified Personal Support Workers (PSW) and Developmental Service Workers (DSW) assist our clients with activities of daily living (ADLs) such as bathing, grooming, medication reminders, mobility assistance. These services help seniors maintain their independence and enhance their quality of life.

Housekeeping Services

Trained in maintaining safety standards in the home and engaging in compassionate care, our housekeeping personnel are able to complete light house cleaning, laundry, organization, de-cluttering and other household chores. There services can provide relief to a busy family and to ensure a clean and comfortable environment for seniors.
